• BOAST 13
• Initiated with in 24 hrs of admission
• Include: Diagnosis, treatment, management plan, transfer/discharge plan, medication, thromboprophylaxis, goals, therapy requirements, outpatient visit, wound care, and referral for further care.
• this is communicated to patient and kept in patient record.
• Designated coordinator
• Functional return should be discussed
