PCL Injury

PCL

  • IA ligament: Intracapsular but extrasynovial
  • Strongest in knee
  • Primary restraint to posterior translation
  • AL , PM bundles and MF ligaments
  • Origin from PL MFC and insert to central posterior tibia 1 cm below articular surface
  • 38 cm , 13 mm Diameter

MOI

  • Direct injury with foot plantar flexed and knee 90 degree flexed
  • Associated with periarticular and SOF #
  • Associated with PLC injury

Diagnosis

  • Tibial step off sign
  • Posterior drawer test
  • Quadriceps active test
  • Dial test ER in
    • 30 degree: PLC
    • 90 degree: PCL and PLC
  • In c/c injury MRI is positive only in 50%
  • Lateral stress view radiograph is Gold standard

Management

  • Isolated PCL: little long term instability
  • Surgery
    • Asso with PLC/ MPC injury/meniscus
