Purpose

The rotational profile is a structured clinical assessment of lower-limb rotation in children with intoeing or out-toeing.

Components

1. Foot progression angle

Observe the angle between the long axis of the foot and the direction of walking.

  • Negative / inward angle indicates intoeing
  • Positive / outward angle indicates out-toeing

2. Hip rotation

With the child prone and knees flexed to 90°, assess internal and external rotation of both hips.

  • Increased internal rotation with reduced external rotation suggests increased femoral anteversion.
  • Femoral anteversion normally decreases with growth.

3. Thigh-foot angle

With the child prone and knee flexed to 90°, compare the axis of the thigh with the axis of the foot.

  • Internal angle suggests internal tibial torsion.
  • External angle suggests external tibial torsion.

Internal tibial torsion is common in toddlers and usually remodels spontaneously. External tibial torsion tends to become more apparent with growth.

4. Transmalleolar axis

The relationship between the knee axis and malleolar axis provides another estimate of tibial torsion.

5. Foot shape

Assess for metatarsus adductus, skew foot or other intrinsic foot deformity.

Rotational alignment changes throughout childhood. Interpretation therefore requires age-specific normal ranges rather than a single adult reference value.

Common patterns

Intoeing

  • Metatarsus adductus
  • Internal tibial torsion
  • Increased femoral anteversion

Out-toeing

  • External tibial torsion
  • Femoral retroversion
  • Foot deformity

Management principles

Most physiological rotational variants improve with growth and require reassurance only. Consider surgical derotation in an older child with severe deformity, persistent functional limitation and measurements well outside the expected range.
